Registration Form with javascript only for validation...HELP...please...

    hi,

        please can anyone give me the source code for making a registration form with javascript for validation. I'm completely lost...   The form should include the following fields:

    1/ Surname

    2/Other Names

    3/E-mail address

    4/Password

    5/Confirm Password

    6/Credit card Number

                   Please help me out i'm completely lost... Thanks in advance...
