Form within a form

  I have created a customer database where a form is used to input customer data.  Each customer has a customer ID and on the form it has a box to add a comment to that customer which will bring up another form to add a comment.  When i enter the comment which should be a 1 to 2 relationship on customer id from the first form to the comment form, it keeps that same comment for all customer id's.  Where do i need to make a change on properties to not have the comment for all customer id's
  I would greatly appreciate any suggestions.
